Bias Incident Reporting Form

The Massachusetts Anti-Bullying Law (G.L. c. 71, § 37O) and Student Anti-Discrimination Act (G.L. c. 76, § 5) clarify that schools must act swiftly to address bias and hate incidents, including bullying and harassment. When a school receives notice of an alleged bias or hate incident, it has a legal responsibility to investigate and respond to the incident. If you are a victim, witness, or hear about a bias or hate incident, 1) promptly seek support from a trusted adult such as a teacher, school administrator, or parent/guardian, and 2) formally report the incident using the designated bias incident reporting form provided by your school below. Expect acknowledgment of your bias incident report within one school day from the school Principal or their designee. Click here to learn more about our district's Bias Incident Response Protocol.
